Hospitalized Patient Care Coordinator
Location: 
Monroeville
Schedule: Full Time | 4 Ten Hour Shifts Per Week | 8am-6pm | Includes One Weekend Shift

Compensation: Up to $22/hr, Dependent on Experience | Applicable Weekend Shift Differentials + $2.50/hr from 12am Saturdays through 12am on Mondays

In Your Day-To-Day, You Will:
  • Support the care experience for hospitalized ER and Critical Care patients by helping keep communication, documentation, and discharge activities organized and moving forward.
  • Connect with pet owners following discharge to check in, answer appropriate questions, and help ensure a smooth transition home.
  • Coordinate the timely communication of laboratory results to pet owners and referring veterinarians following doctor review and authorization.
  • Help facilitate patient discharges by reviewing discharge instructions with clients and preparing take-home medications.
  • Serve as an important communication link between doctors, hospital teams, pet owners, referring veterinarians, and other external partners.
  • Review patient charges throughout hospitalization to help ensure services and treatments are captured accurately and completely.
  • Identify missing, duplicate, or inaccurate charges and partner with doctors and department leaders to resolve discrepancies.
  • Track outstanding laboratory results, client and referring veterinarian communications, discharge needs, and other follow-up items through completion.
  • Maintain clear, accurate, and timely documentation in the patient's electronic medical record.
  • Prioritize responsibilities based on patient needs, anticipated discharge times, pending communications, and the changing demands of a busy ER and Critical Care environment.
  • Process outside pharmacy requests in accordance with hospital protocols and doctor authorization.
  • Collaborate with team members across the hospital to improve workflow, communication, and the overall experience for patients, clients, and referring veterinary partners.
What You Bring:
  • A high school diploma or equivalent.
  • Previous experience in a veterinary, medical, or clinical support environment is preferred, along with a basic understanding of veterinary medical terminology.
  • Strong communication skills and the ability to interact professionally and compassionately with pet owners, doctors, hospital team members, referring veterinarians, and external partners.
  • Excellent attention to detail—you take pride in making sure information, documentation, and charges are accurate and complete.
  • Strong organizational and time-management skills, with the ability to keep track of multiple follow-up items and see tasks through to completion.
  • The ability to prioritize and adapt in a fast-paced hospital environment where patient needs and priorities can change quickly.
  • Comfort using email, Microsoft Office/Office 365, and electronic medical record systems, with the ability to learn new technology and hospital systems.
  • Good judgment and professionalism when handling sensitive patient, client, and hospital information.
  • A collaborative, team-oriented approach and a willingness to step in and support others when needed.
  • A commitment to providing an excellent experience for patients, pet owners, referring veterinary partners, and your fellow team members.
