Discharge Coordinator overall role: The role of Discharge Coordinator entails many duties to help ensure clear communication between our hospital and clients (owners and PCVet) and internally within CVRC. They also assist doctors with client communication, communicating and assisting clients that have questions after discharge, as well as facilitating an efficient and clear transition from in hospital care to continued and or home care for clients. Additionally, the discharge coordinator helps support the maintenance of patient records and client communications such as laboratory and radiology results, call backs, and follow ups. The discharge coordinator also reviews in hospital billing and will audit as needed if any discrepancies.
Education and experience requirements:
High School Diploma or G.E.D.
Basic math, reading and writing skills
Compensation:
Starting at $22-$24/hr
Schedule:
Primarily Sunday-Thursday
Overnight with some flexibility required
Preferred experience:
3+ years’ experience of experience working in a team of 10 o rmore.
3+ years’ experience working in a specialty veterinary environment
Bachelor’s degree in relevant discipline
Physical requirements:
Able to be active: sitting, walking, standing for 8 hours or more a day
Able to work well under pressure and at high levels ofstress
Able to lift 40 pounds
Flexible with work hours

Essential Functions and Responsibilities: General Tasks:

Maintain strict confidentiality regarding clients and patients for whom the practice provides veterinary services
Be able to learn the medical records system and use accurately to maintain medical records and invoicing

Maintain professional and accurate interdepartmental communications regarding patient and client issues.
Take messages for team members and ensure that they are given to them in a timely manner
Promote open communication regarding daily practice operations
Maintain inventory of food samples, pill cutters, and discharge folders
Specific Client Tasks:
Update discharge information in appropriate PIM system when updated by doctor
Facilitate scheduling all discharges
Ensure all discharging tasks and medications are filled and prepared before scheduled discharge
Triage and field all client questions and concerns
Assist with maintaining lab call back sheets in doctor’s station
Communicate effectively with PCVet
Medical Record Tasks:
Have awareness of CSR support needs
Answer phones
Ensure triage clipboards are readily available
Clean rooms/lobby/mop as needed
Print patient stickers
Collect completed patient clipboards and organize for scanning
